LVN Intake Coordinator
Job Description
Location: Clearlake, CA
Job Summary:
Coordinates incoming office referrals and verifies insurance coverage. Coordinates incoming calls from patients, family, physicians, and disciplines. Ensures that all personnel are informed of patient's hospitalization and follow-up with patients admitted to other hospitals. Coordinates small program(s) with limited budget/impact. Applies substantial knowledge and experience to perform a wide range of advanced activities and/or determines how to use resources to meet schedules and goals; serves as working supervisor for team or work group.
Schedule:
- Day Shift. 13 Week assignment
- Monday – Friday
- 8:00am – 5:00pm
Pay:
- $27.50 DOE
Essential Job Functions:
- Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions, and patient referrals.
- Will be a clinical intake coordinator for home health processing and scheduling referrals.
- Facilitates telephone communications between, and demonstrates effective communication skills, with nursing, therapy, aide, social services, physicians, regarding changes in patient/staff schedule, test results, etc.
- Assists with staffing/scheduling activities, soliciting input from managers.
- Provides follow-up for hospitalized patients, ensuring subsequent agency referral.
- Performs other job-related duties as assigned.
Requirements:
- Must have a CA LVN certification
- Must be able to pass medical screenings
- Must be able to pass background check.
- Must have a high school diploma.
- Must have 1 year of experience
- All associates receive all required vaccinations as a condition of employment and annually thereafter, where applicable. Medical and religious exemptions may apply.
