Work Schedule: Monday to Friday, 8am - 5pm
Hire Up Healthcare is currently seeking a skilled Licensed Social Worker (LCSW) for a Direct Hire opportunity in Bakersfield, California. This outstanding prospect entails partnership with a distinguished hospital and mandates a minimum of 2 years of experience of relevant experience in JOB.
Responsibilities:
- As a Licensed Clinical Social Worker (LCSW), you will play a vital role in providing comprehensive support and care to individuals and families. Your responsibilities will include:
- Development and Implementation: Contribute to the development and execution of Quality Assessment and Performance Improvement (QAPI) activities to ensure the highest standards of care.
- Interdisciplinary Collaboration: Collaborate effectively as a member of an interdisciplinary team, participating in initial, semi-annual, unscheduled, and annual assessments to create personalized care plans.
- Individual and Family Counseling: Offer individual and family counseling as required or as specified in the care plan, delivering compassionate and effective therapeutic support.
- Group Counseling and Support: Design and lead group counseling and support sessions, fostering a sense of community and providing valuable insights to participants.
- Liaison Responsibilities: Act as a liaison between participants and external agencies such as the Department of Aging, Social Security Administration, Medicaid, and more, ensuring seamless access to necessary resources.
- Crisis Intervention and Advocacy: Provide timely crisis intervention services and advocacy, ensuring that participants receive the support they need during challenging times.
