Job Description
Clinical Care Coordinator (LVN OR RN license/education required) – Fresno, CA (In-Office Only)

Location:Â Fresno, CA

Schedule: Monday through Friday, standard business hours

Work Environment: In-office, not patient-facing — communication is over the phone and through Teams meetings

Pay: $33/hour to start (range: $30–$36/hour DOE)

Position Type: Full-time, hourly
Job Overview:
-
Coordinates post-discharge care and complex case management for patients to reduce readmissions and improve outcomes

Works closely with physicians, specialists, and community providers
-
Maintains accurate documentation for care coordination and reporting

Uses a blend of clinical judgment and communication skills to support patients and healthcare teams
Requirements:
-
RN or LVN license in the State of California
-
Graduate of an accredited nursing program
-
Minimum 2 years of nursing experience in a hospital or clinic setting

Strong communication, computer, and critical thinking skills
This is an excellent opportunity for a nurse who enjoys case coordination without direct patient care, in a structured and collaborative office setting.
